New Patients Please Note:
If you are transferring from another provider, it is best to have access to previous records (but not required). Also, the provider is not required to continue the current treatment plan from a previous provider if that treatment plan does not align with best practice and the mission of Ivy Health & Psychological Services.

Rates

$250 Initial Visit/New Patient

$125 Follow-Ups

All major credit cards and cash are accepted.

For virtual appointments, a credit card must be on file before scheduled appointment.

Co-pays (if applicable) are due at time of service.

Payment arrangements are available for self-pay clients if needed.

Practice          Policies

Cancellation Policy:
If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you will be required to pay a $25 no-show fee.

Insurance Verification:
Although we may be in-network with your insurance carrier, we strongly urge each patient to verify your plan does afford coverage for services rendered.  Please reach out to our staff with any specific questions you may have regarding insurance.

Paperwork or Letters:
There is a $10 fee for paperwork and letters that need to be completed by the provider.

FMLA or Disability paperwork will not be completed at the first visit. You will need to be patient of IPHS for
at least 6 months.
